Health Insurance Enrollment
All employees who have HMO coverage that will not be offered in the upcoming September 2000 enrollment must complete an enrollment form to sign up for the State Health Plan or another HMO (if appropriate). There will not be an automatic roll over into the State Health Plan or an alternative HMO.
HMO’s that will no longer be offered effective October 1 are: Personal Care Plan, Optimum Choice, Partners, QualChoice, Wellness Plan and United HealthCare.
Employees who do not have a change in their coverage do not have to take action.
BCBS Dental Plan Enrollment Is Now
Blue Cross Blue Shield Dental Plan I open enrollment for DOC employees is going on now. Anyone enrolling during open enrollment will have a 12- month waiting period for all services except preventive and diagnostic services.
If you have not received an enrollment form and would like to, please call Doug Sutton’s office at 1-800-788-7771.
Completed applications must be returned to Doris Martin, DOC payroll office, by July 28. Coverage will be effective September 1 following the August paycheck deduction.
New BCBS Dental cards were mailed in April to employee home addresses. Many have been returned. Please obtain change of address forms from your benefit representative.
