Chronology Related to Keith Moore Incarceration

 

Sept. 28, 2000 -- Moore admitted to Central Prison following a probation violation on an embezzlement conviction from Wilson County

Sept. 29, 2000 -- Central Prison involuntarily admitted Moore to inpatient mental health treatment unit because of previous behavior in community and he was placed on suicide watch

Oct. 3, 2000 -- Moore removed from suicide watch after mental health professionals observe Moore’s behavior for several days and assess his mental status. He denied suicidal intent.

Oct. 9, 2000 -- Moore released from inpatient treatment unit after further observation. He was transferred to Unit 1, Administrative Segregation.

Oct. 9 - Nov. 29, 2000 --  Mental health staff sees Moore for supportive therapy seven times on             an outpatient basis; Moore expressed no suicidal intent.

Nov. 29, 2000 -- Mental health staff meets with Moore. Follow-up appointment scheduled for Nov. 30 to review results of Moore’s appearance before the Facility Classification Committee to discuss future classification and housing placement.

Nov. 30, 2000 -- At approximately 4:35 p.m., Moore found hanging in cell of Unit 1, C Block housing area
