Prison Program Description: Case Management | |
Purpose | Case management and correctional counseling are utilized to coordinate and provide services to each inmate during his or her stay in prison. This is achieved by planned contact and interaction between correctional staff and inmates. |
Criteria for Participation | All inmates are assigned a Case Manager. |
Program Operation |
The case
management process establishes a system by which correctional staff assists
and guide inmates in making an adjustment to confinement and preparing for a
successful reentry into the community. The process utilizes a case management
assessment to identify the inmates areas of need. Upon completion of the
assessment a case management plan is developed to address the needs and
guide the delivery of programs and services throughout the inmate’s period
of incarceration. These services are delivered by trained staff that
provide correctional counseling, crisis intervention and classification and
program reviews.
